CLAIM DETAILS
1) Claim is payable only when the mobile is used by the person mentioned in the Invoice during purchase of the Mobile ( find exhaustive list of exclusions above)
2) Claim is paid only by means of an account payee cheque in the name of the Insured person, namely the person named in the Invoice. 3) The Insured is expected to act prudently, while taking care of the insured mobile, as otherwise the claim is likely to be rejected.
4) Claim cheques are sent to the address mentioned in the claim form by courier. 5) In order to avoid return of correspondences and cheques due to inadequate / improper address, Insured are advised to give their complete correspondence address along with contact phone number in the claim form. 6) Geographical area of Insurance coverage is INDIA.

Documents Required
All Claims:-
1) Claim form fully filled in and signed, Original Invoice Form, Original Proposers Detail Form 2) Letter giving detailed description about how, when & where the loss or damage took place.
Additional Documents Required in case of
Repairs:-
1) In case the phone is repairable, an estimate with details of kind of repair or replacement of parts needed with the likely cause and the corresponding amount for the same, should be obtained from the service engineer. The estimate and mobile has to be shown to the above mentioned Policy Issuing Office for their verification & approval.
2) Original Repair Bill with details of parts replaced & amount, labour charges, Name of the Owner / Insured, IMEI No, Model.
3) In cases of total board replacement , the new IMEI no with surrender of the old board.
Total Loss, where the Mobile is not repairable:-
1) In case the phone is not repairable, an estimate of the likely cost to repair and a certificate to that effect (not repairable) from the service engineer.
2) The entire mobile with battery to be surrendered. ( Small deduction is made in case battery is not surrendered )
Total Loss, where the Mobile is lost due to Theft or by Fortuitous circumstances (happening by chance) :-
1) Copy of Complaint letter acknowledged by Police authorities / CSR or Complaint received certificate.
2) Non Traceable Certificate -NTC
3) Ensure that Name, Address, Date of Loss, IMEI No , Model, CSR or Complaint No are mentioned in the NTC.
4) Proof of either the lost phone no has been blocked / barred (or) the same phone no has been obtained again.
5) Letter of Subrogation in Rs.100/- non-judicial stamp paper signed by Insured / Owner with name & address, sign of one witness with name & address . Every page is to be attested by a Notary Public with his sign & seal.
Note: One may use bond sheet (as second page) to continue typed matter from stamp paper (first page) (or) use two Rs.50/- stamp paper.

Compensation Payable
1) Repairs:-
Charges towards parts and labour. ( Including VAT , Service Tax with proper receipt )
Less depreciation according to the age of the mobile on the cost of parts Less deduction for under insurance ( when sum insured is less than market value at the time of loss, claim is deducted proportionately ) ( most of the claims do not fall under this category, so there will be no deduction in this respect )
Less Policy Excess of Rs.250/- ( for policies issued from May'10 and thereafter) / Rs.500/- (for policies issued before May'10) Less Re-instatement premium ( premium towards reinstating the sum insured to its original amount calculated on the amount of net claim payable)
( very small amount, hence, net claim amount largely remains unaffected ) Note: Nominal deduction is also made when salvage (old part) was required to be surrendered but was not done, usually when the whole board is replaced etc. For smaller parts replaced, surrender of salvage is not insisted upon.
2) Total Loss due to Theft, Fortuitous Circumstances (happening by chance), Not Repairable:-
Sum Insured or Market Value which ever is less on the date of loss. Less deduction for under insurance (sum insured is less than market value at the time of loss) ( most of the claims do not fall under this category, so there will be no deduction in this respect ) Less Policy Excess of Rs.250/- ( for policies issued from May'10 and thereafter) / Rs.500/- (for policies issued before May'10) Note: Nominal deduction is also made when salvage (old part) was required to be surrendered but was not done. Eg. In case of Total Loss (not repairable) surrendering the Instrument with out the battery.

Terms
Depreciation Means:- Deduction made towards the age of the mobile ( No of days from date of purchase to date of loss/damage). 5% to 50%
Market Value Means:- Value of new Mobile (same model) on date of loss
less deduction towards accessories less deduction towards depreciation Value is adjusted / reduced for obsolete ( no longer in use ) models.
Sum Insured Means:-The value for which the Mobile has been Insured for. Mostly the Invoice price.
